Patient Forms

You will need to complete our new patient packet. The packet can be downloaded, printed and completed before you arrive at our office. For the first appointment, we suggest you arrive 10 minutes early with your completed forms and 20 minutes early if your forms are not completed. If you have any questions when filling-out these forms, our helpful staff will assist you when you arrive for your appointment.

If you do not have Acrobat Reader you can get it for free by clicking on the 'Get Acrobat® Reader' icon. This will take you to Adobe's website and enable you to download the program to your computer.

Payment Policy

Payment is required for all services performed at the time of service, unless you are in a prepaid plan in which we participate. For those patients, co-payments, co-insurance and deductibles will be collected. The patient is responsible for any and all charges not paid for by their insurance company. For out-of-network and uninsured patients our office will collect the cost of services performed at the time of service. For your convenience, we accept cash, check or credit card (Visa, MC, and Discover).

If you must cancel or reschedule an appointment, please do so at least 24 hours before the scheduled appointment time. Your courtesy to Dr. Nguyen and his staff, as well as the other patient for whom an appointment could have been used, is greatly appreciated. Our office may charge a late fee to patients who miss their appointment or do not notify our office of cancellation 24 hours in advance.

Insurance Policies

We accept appointments for patients regardless of their insurance status.

Dr. Nguyen is currently a provider for:

  • Medicare and TRICARE
  • All major Southern California PPO insurance carriers (i.e. Anthem Blue Cross, Blue Shield, Aetna, Cigna, Health Net, United Healthcare/Pacificare, etc.)
  • TRICARE HMO
  • Greater Tri Cities IPA

* Our office does not accept Cal-Optima/MediCal/Medicaid/ or any HMO insurances except Tricare and Greater Tri Cities IPA.

* Our office does not participate or do any administration or documentation for Worker's Compensation claims.

Patients are responsible for checking our participation with their insurance plan before their visit. Patients are responsible for any/all charges not paid by their insurance company. For out-of-network or uninsured patients, our office will collect the cost of services performed at the time of service.

Patient Privacy & HIPAA Compliance

It is the policy of our office that physician and staff preserve the integrity and the confidentiality of our valued patients. Compliance with the Health Insurance Portability and Accountability Act (HIPAA) ensures that we protect our patient privacy while having the necessary medical information to provide the highest quality medical care possible. Our office requires your written acknowledgement of the review of our privacy notice on how our office may use protected health information.
